Patient Consent for Telehealth

Last updated October 6, 2026

Please read this before you join. When you check the consent box at checkout, you confirm you’ve read it, that you understand it, and that you agree to it.

1. What telehealth is

Telehealth means receiving care from a licensed provider remotely, by video, by phone, or through a secure online health questionnaire and messaging, instead of in person. Your provider decides which of these is appropriate for you.

2. Who provides your care

Your care is provided by independent licensed medical providers through Curate Health’s telemedicine partner. Curate Health isn’t a healthcare provider and doesn’t diagnose, treat, prescribe or dispense. Your provider makes every medical decision, including whether any medication is appropriate for you, and may decide it isn’t. Your provider may also ask you to agree to their own consent at your visit.

3. Benefits

Telehealth can make care easier to reach: no travel, less waiting, and visits from wherever you are.

4. Risks and limits

A provider can’t examine you in person. Decisions rely on the information you give, so incomplete or inaccurate information can lead to the wrong decision. Technology can fail and interrupt or delay a visit. Your provider may decide that telehealth isn’t right for you and that you need to be seen in person. Despite strong safeguards, no electronic system is entirely free of the risk of a privacy or security breach.

Any medication prescribed carries its own benefits and risks, which your provider explains. Compounded medications are prepared by a licensed pharmacy and aren’t approved by the FDA.

5. Emergencies

Telehealth through Curate Health isn’t for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.

6. Your responsibilities

You agree to give complete and accurate information about your health, your medical history and the medications you take, to tell your provider about any change, to follow their instructions, and to keep seeing your own primary care doctor for your general health.

7. Privacy and records

Your visits are conducted on secure systems, and your medical records are kept by your provider under federal and state law. How health information is handled is explained in our HIPAA Notice and Privacy Policy.

8. Your choices

Agreeing to telehealth is voluntary. You may withdraw this consent at any time by emailing info@curatehealth.clinic, which ends future telehealth care through Curate Health; it doesn’t affect care you’ve already received. You must be 18 or older and located in a state where your provider is licensed at the time of each visit.

9. Your consent

By checking the consent box at checkout, I confirm I’ve read this Patient Consent for Telehealth, that I’ve had the chance to ask questions, and that I consent to receiving care by telehealth from independent licensed providers through Curate Health’s telemedicine partner.
